Carrier Class Services

Analog Line Bundle
- Analog Lines are generally delivered over copper T1 or fiber with conversion to
TDM lines. This product is typically used to provide service to landlines, fax
machines, alarm systems, etc. Ten lines minimum required for this product. Offered
in 24, 36, and 60 month term options.
PRI
- Provides up to 23 simultaneous calls (23B+D), and connects physically with the
Public Switched Telephone Network to provide high quality of service. 100 DIDs are
included with this product. Offered in 24, 36, and 60 month term options.

DIDs
- Additional DIDs for use with our PRI product. Offered in 24, 36, and 60 month term
options.

Remote DIDs
- Foremost also offers Outbound- only remote DIDs within specific LATAs. Offered in
24, 36, and 60 month term options.

Emergency Location Identification Number
- Provides specific, customized 911 information for emergency responders when 911 is
dialed, allowing for DIDs to be configured to show a different address for
configurations where PRI/SIP services are delivered to a central location and
distributed to multiple sites. Up to 100 locations are included with this product.
Offered in 24, 36, and 60 month term options.

Remote Call Forwarding
- Forwards inbound calls from their original destination to a different destination.
Service pricing varies based on whether this is local, remote within LATA, or long
distance. Offered in 24, 36, and 60 month term options.

Caller ID Name
- In addition to seeing an inbound call’s phone number, the name of the business or
person calling is also provided. Offered at a 24 month term commitment.

Directory Listings
- Directory Listings register the customer name, number, and other associated
information in the Directory Assistance database.

Accounting Codes
- Account codes track long distance usage by individual user calling long distance.
Allows management to locate specific abuses of long distance calling, and only allow
specific parties to call long distance. Can also include unverified codes that don’t
require a password to call long distance for general use.

Long Distance Services
- Foremost offers a variety of different 48 State Outbound Long Distance rates and
plans to meet the needs of any client. Our options include the following.
o Long Distance ad hoc 48 State Interstate
o Long Distance ad hoc 48 State Intrastate
o Long Distance 48 State 5,000 minutes plan
o Long Distance 48 State 10,000 minutes plan
- Foremost also offers the following switched and dedicated 48 State Outbound Long
Distance services.

o Long Distance 48 State 5,000 minutes plan, at 24, 36, and 60 month term options.
o Long Distance 48 State 10,000 minutes plan, at 24, 36, and 60 month term options.

Toll Free Number
- Allows any person to call in toll-free to your site. Foremost offers the following
options in toll free number services.
o Toll Free Numbers, provided at a flat MRC plus usage, in 24, 36, and 60 month term
options.
o Toll Free Long Distance ad hoc 48 State Interstate
o Toll Free Long Distance ad hoc 48 State Intrastate

Managed PBX Services
- Provides your location with the flexibility and customization of a PBX system,
without the overhead of purchasing, storing, and maintaining your own equipment.
Instead, Foremost provides the services of the PBX through an internet connection to
you, and handles the maintenance as well. Foremost offers the following levels of
service for our Managed PBX platform on a per user basis. Note that this item does
NOT include a SIP phone and internet service, which are required to make use of the
Managed PBX Services.
o Platinum Services: Our highest level of service for users that need the most out
of their Managed PBX service. Includes the following features: Call Logs, Call
Waiting, Caller ID, Caller ID Delivery Blocking, Client Call Control, Privacy, 10
Digit Telephone Number (DID), Call Forward Always, CF Busy, CF No Answer, CF Not
Reachable, Call Hold, Call Return, Call Transfer, Do Not Disturb, Last Number
Redial, Message Waiting Indicator, 3 Way Calling, Voicemail, Voicemail to Email,
Anonymous Call Rejection, Anywhere Log In, Call Block, Directed Call Pick Up, Find
Me/Follow Me, Hoteling, Multiple Call Arrangement, N-Way Calling, Intercom,
Selective Call Acceptance, Shared Call Appearance, Speed Dial, and Priority Alert.
Offered in 24, 36, and 60 month term options.
o Gold Stations: Our recommended level of station for most users using the Managed
PBX Services on a day-to-day basis. Includes the following features: Call Logs, Call
Waiting, Caller ID, Caller ID Delivery Blocking, Client Call Control, Privacy, 10
Digit Telephone Number (DID), Call Forward Always, CF Busy, CF No Answer, CF Not
Reachable, Call Hold, Call Return, Call Transfer, Do Not Disturb, Last Number
Redial, Message Waiting Indicator, 3 Way Calling, Voicemail, Voicemail to Email.
Offered in 24, 36, and 60 month term options.
o Silver Stations: The most minimalistic station option. Great for break room
phones, side rooms, or other locations where you want a phone connected to the
network without a dedicated user. Includes the following features: Call Logs, Call
Waiting, Caller ID, Caller ID Delivery Blocking, Client Call Control, Privacy, 10
Digit Telephone Number (DID), Call Forward Always, CF Busy, CF No Answer, CF Not
Reachable, Call Hold, Call Return, Call Transfer, Do Not Disturb, Last Number
Redial, Message Waiting Indicator, 3 Way Calling, Voicemail, Voicemail to Email.
Offered in 24, 36, and 60 month term options.

Hosted PBX SIP PSTN Services
- This service provides the underlying voice connectivity for the Hosted PBX
stations and is required in order to use a Hosted PBX product for external calling.

Hosted PBX Autoattendant
- A fully customized automatic attendant system for your PBX. Allows routing of
customers to different departments using key presses, joining conference bridges,
playing audio messages, and other features. Configuration is initially performed by
Foremost at a rate of $250 per hour. Customer should provide audio files and other
assets as desired. Following configuration, the service is maintained on the Managed
PBX system for a MRC at 24, 36, and 60 month term options.

Hosted PBX SIP Phone Lease
- Foremost leases Yealink brand SIP phones for use alongside our Managed PBX
services. We offer several different options intended for different levels of use.
o Basic Phone Lease: Recommended for Silver users. Provides a Yealink T21 or similar
device at 24, 36, and 60 month term options.
o Advanced Phone Lease: Recommended for Gold users. Provides a Yealink T40 or
similar device at 24, 36, and 60 month term options.
o Executive Phone Lease: Recommended for Platinum users. Provides a Yealink T46 or
similar device at 24, 36, and 60 month term options.
o Receptionist Phone Lease: Recommended for Platinum users, or other users dealing
with phone traffic for the majority of their responsibilities. Provides a Yealink
T48 or similar device at 24, 36, and 60 month term options.
o Receptionist Sidecar Lease: Provides a EXP 40 or similar sidecar device allowing
the user to quickly dial many different numbers easily and efficiently at 24, 36,
and 60 month term options.
Hosted PBX Custom Features
- Foremost supports advanced level customization of the Managed PBX system per
customer direction at a charge of $250 per hour.

SIP Trunking
- Provides voice services digitally to PBXs and other systems. Offered at 24, 36,
and 60 month term options.

Voice Mail
- A voice mail box to store audio message from missed calls that you can retrieve
later. Offered per path at 24, 36, and 60 month term options.

Conference Bridge
- A conference bridge for multiple users to call into for meetings and other events
requiring several people all being on the same call at the same time. Offered per
path at 24, 36, and 60 month term options.

Dedicated Internet Access
- Dedicated Internet Access direct to your location. This is a high tier service
with no bandwidth sharing or other over-subscription practices interfering with your
connection. This service includes transport. Provided in a wide range of speeds from
50 Mbps to 40 Gbps. Rates vary depending on location proximity to the Foremost Fiber
Network. Offered at 24, 36, and 60 month term options.



IP Addresses
- Static IP addresses that provide a consistent address reachable through the
internet. Provided at /32, /30, and /29 block options.

Managed Communication Services
- Foremost offers Managed Communication Services at different rates based on the
expertise and scope of work required. Rates are also subject to change based on
whether labor is performed Onsite (at the customer location) or Onshore (at the
service provider location). The following categories are offered.
o Category 1: One technician performing cabling, data entry, equipment installation,
or general troubleshooting.
o Category 2: Two technicians performing cabling, data entry, equipment
installation, or general troubleshooting.
o Category 3: One technician performing upper level configuration, troubleshooting,
and equipment installation.
o Category 4: One technician performing upper level configuration, troubleshooting,
equipment installation, and scripting as needed.
o Category 5: One technician performing design, scripting, and software development.

desired time frame for delivery. If there are any special terms required, please
also include that information.

If a quote is accepted, a PO can be issued. This would be provided to the Foremost
team member addressing your quote request. However, please be aware that Foremost
requires the Foremost Master Service Agreement and a Foremost Service Order to be
executed as a component of providing service.
